Who Qualifies For C-SNP?
C-SNPs are specifically targeted to meet the healthcare needs of their plan members who have chronic or disabling conditions. A chronic condition is considered to be a condition that lasts for a year or more and causes the individual to receive ongoing medical care.
To qualify for a C-SNP, you must have one of the following 15 chronic conditions:
- Severe hematologic disorder
- Stroke
- Neurologic disorder
- Chronic and disabling mental health condition
- Chronic heart failure
- Cardiovascular disorder
- Cancer
- Dementia
- HIV/AIDS
- Diabetes mellitus
- Autoimmune disorder
- End-stage renal disease
- End-stage liver disease
- Chronic lung disorder
- Chronic alcohol or drug dependence
Since C-SNPs are designed to cater to your specific healthcare needs, you will essentially pay very few out-of-pocket costs. All C-SNPs are also required to provide their plan members with prescription drug coverage.
How To Enroll
If you meet the requirements for a C-SNP, you can enroll at any time as you will qualify for a Special Enrollment Period. The Special Enrollment Period is a period that exists outside of the regular enrollment periods that are only available to those who have gone through very specific situations, such as having a chronic condition and needing to enroll in a C-SNP that serves your condition.
