What Is Medicare?

Medicare is the federal health insurance program that provides coverage for those who are 65 or older, younger people with disabilities, and those diagnosed with End-Stage Renal Disease.

What Is Medicaid?

Medicaid is a program jointly funded by state and federal governments, and it covers people with low incomes and limited resources.

Who Qualifies For D-SNP?

As long as you meet the eligibility requirements for both programs, you will be eligible to enroll in a D-SNP. Having a D-SNP can actually be very beneficial when it comes to the health coverage benefits and care coordination. Some D-SNPs will even provide additional coverage benefits, but this will vary by plan and carrier. These additional benefits can include:

  • Prescription drug coverage
  • Dental, vision, and hearing coverage
  • Transportation to doctor appointments
  • And more

When Can I Enroll?

Once you know you are eligible for a D-SNP, you can begin your enrollment process during regular enrollment periods, such as:

  • Initial Enrollment Period – This period starts three months before the month of your 65th birthday and will end three months after your 65th birthday. 
  • Annual Enrollment Period – From October 15 to December 7 of each year spans the Annual Enrollment Period. During this time, people who are enrolled in Medicare Advantage or a Medicare Part D plan can drop their plans and return to Original Medicare, renew their plan contracts, or switch to a different plan.
  • Special Enrollment Period – Special Enrollment Periods don’t have a specific time of year of when they occur as they are only available for people who meet certain conditions, such as becoming eligible for both Medicare and Medicaid.
